Hazard Reporting at the ANU

Hazard elimination and control is the primary focus of our OHS injury and disease prevention programs.   The reporting mechanisms are designed to encourage the early detection and correction of hazards to prevent staff, students or visitors being injured or harmed.  In general, the hazards are divided into –

Dangerous Occurrences –   means an occurrence that could have caused a Death, Serious Personal Injury or Incapacity.  It is an event that did occur.  For example, an electrical appliance becoming live that could have caused an electrocution.

To report a Dangerous Occurrence use the incident notification form.  These hazards are generally reported to COMCARE within a set timeframe.  The OHS Unit, Budget Unit management or HSR, conducts an investigation into the incident.

(other) Hazards -   these are situations that may currently exist or could arise out of a set of circumstances.  That is there is a potential risk that the hazard may lead to a dangerous situation.  (Dangerous) Occurrences of lesser significance\potential may also be reported through this process.  For example, a permanent electrical cord running across the floor of a walkway. 

The Hazard Report form is used by the local Budget Unit to address these hazards.  The Hazard Report form gives employees and students a convenient method whereby, having identified a potential hazard, they can communicate this information to their supervisor for review, transmission to the Budget Unit manager, and action to eliminate or control the hazard.

The three stages of entry onto the report ensures input from the supervisor and manager and a reply from line management to the person initiating the report:

STAGE 1:

To be completed by an employee or a student who is concerned about a potential hazard. 

Visitors wishing to report a hazard within a building should approach the business office or a staff member. 

Campus hazards should be directed to Facilities and Services, Client Services Unit on Ph 6125 4000.

STAGE 2:

To be completed by the immediate supervisor of the originator (or Health and Safety Representative) and submitted to the Business Manager (or equivalent) of the Budget Unit.

STAGE 3:

To be completed by the Business Manager indicating the action to be taken with relevant reasons and return a copy to the

  • Person reporting the hazard
  • Occupational Health and Safety Unit
  • Designated Work Group OHS Committee
